Statement of Informed Consent for a Usability Session

A group of Masters students in the School of Computer Science, Telecommunications and Information Systems at Depaul University are conducting a study to assess the usability of a web site that will sell jewelry. If you volunteer to participate in this study, you will be asked to work with a prototype of the website, navigate through the site, enter some information into the site, and answer some questions. Questions will be asked in a web questionnaire provided at the end of the session.

Your interactions with the site will be recorded by the server. You may skip any tasks as well as quit the session at any time by either closing the browser window or clicking on a "quit" button when available

There are no direct benefits to you for participating other than what may be an educational experience. There should be no risks to you; you will simply be using a web site/web application, entering some information, and viewing and evaluating the results. We will associate an ID number with your experience and will keep a record of the link between your initials and ID number. However, your initials will not be used, if and when we discuss the results in research publications.
